Job Summary: We are seeking a highly skilled and compassionate Telehealth RN Case Manager to join our dynamic healthcare team. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services. Requirements:
  • Current unrestricted RN license in Michigan.
  • BLS Certification required
  • Minimum 2 years of clinical nursing experience, with a background in case management preferred.
  • Certification in Case Management (CCM) preferred.
  • Proficiency in using telehealth platforms and electronic health records (EHR) systems.
  • Excellent communication skills, both verbal and written.
  • Strong critical thinking and problem-solving abilities.
