Job Description

Position Summary:

The Dietitian role has a clinical role maintaining a minimum of 30 minutes per resident per month at our long term care home in Windsor for a total of 70 hours a month.

Responsibilities:

  • Identify and monitor level of nutrition and hydration for all residents.
  • Complete scheduled RAIMDS assessments; assess nutritional status for all residents upon admission, annually and when significant changes in status occur.
  • Prioritize and respond to nutrition referrals in a timely manner.
  • Develop, evaluate and modify nutritional care plans for all residents; liaise with Residents, family members and/or substitute decision makers and the interdisciplinary team.
  • Conduct swallowing assessments, as needed.
  • Provide nutrition counselling and education to residents and family members and/or substitute decision makers, as needed, in line with supporting residents’ nutrition care.
  • Participate in Care Conferences and committee meetings as needed
  • Routine communication with administration, physicians, the allied health team, food services and Nutrition Manager.
  • Review and approve seasonal menus; develop individualized menus for residents, as needed.
  • Provide education and training to dietary and nursing staff, as needed.
  • Participate in home-specific continuous quality improvement initiatives, as needed.
  • Knowledge of the Fixing Long-Term Care Homes Act, 2007 & Regulations and the Resident’s Bill of Rights.
  • Complete other duties, as needed.

Qualifications
  • Current registration in good standing with the College of Dietitians of Ontario required
  • Minimum 2 years relevant experience in a long term care home is an asset
  • Member of the Dietitian Gerontology Network of Canada is an asset
  • Carry Own Professional Liability Insurance
  • A clear Vulnerable Sector Screen is required
  • Computer Literacy - knowledge of computerized electronic medical records (EMR) programs is an asset
  • Swallowing assessment experience would be an asset
  • Ability to work in a team and independently
  • Good verbal and written communication skills
  • Excellent organizational and time management skills
  • Demonstrates ability to problem solve.
  • Strong leadership skills
  • Ability to maintain high professional and ethical standards
