Job description

We are seeking an experienced Registered Nurse Care Manager to provide comprehensive care management services for members with complex physical health needs. This position combines remote work with field-based member visits to ensure individuals receive the care, education, and community resources needed to achieve the best possible health outcomes while maintaining their independence.

This is a hybrid role requiring approximately 50–60% field work and 40–50% remote documentation and care coordination.

Location: Hybrid (Remote work with field visits)

Assignment Length: 6 Months (Potential for Extension or Conversion)
Schedule: Monday–Friday | 8:00 AM – 5:00 PM EST
Location: Remote/Field Hybrid – Must reside in Northeast Ohio

Candidates must reside in Northeast Ohio, preferably near one of the following counties:

  • Trumbull
  • Stark
  • Seneca
  • Portage
  • Mahoning

Responsibilities

  • Develop, implement, and manage individualized care plans for members with complex healthcare needs.
  • Conduct comprehensive in-home assessments and ongoing member visits.
  • Coordinate care with physicians, specialists, community organizations, and other healthcare providers.
  • Educate members and their families on disease management, treatment plans, and available resources.
  • Identify barriers to care and connect members with appropriate services.
  • Maintain accurate documentation and complete visit notes within 24 hours.
  • Monitor member progress and adjust care plans as needed.
  • Meet required turnaround times and performance metrics.
  • Collaborate with interdisciplinary teams while independently managing a daily schedule.

Typical Workday

  • Log in by 8:00 AM EST to review emails, voicemails, tasks, and scheduled appointments.
  • Conduct 5–7 in-home member visits each week.
  • Coordinate care with providers and community resources.
  • Complete all required documentation within 24 hours of each visit.
  • Participate in virtual meetings and team collaboration as needed.
  • Manage your own field schedule while working independently.

Required Qualifications

  • Active Ohio Registered Nurse (RN) License
  • Graduate of an accredited School of Nursing
  • 2–4 years of nursing experience
  • 2–4 years of Case Management and/or Home Health experience preferred
  • Reliable transportation and valid driver's license for weekly travel
  • Strong computer proficiency
  • Experience using:
    • Microsoft Office (Word, Outlook, etc.)
    • Internet Explorer/Chrome
