Position Summary

The Hospital at Home Physician provides acute-level inpatient care to medically appropriate patients in their homes through a hybrid care model that includes in-home visits by nursing staff, virtual rounding, and interdisciplinary coordination. This physician will lead care delivery in a value-based, patient-centered model designed to reduce inpatient utilization while maintaining high clinical quality and safety standards. This role combines traditional hospitalist-level acuity with innovative care delivery, leveraging remote monitoring, mobile diagnostics, and interdisciplinary home-based services.

As a physician in this innovative model, you will:

  • Manage acute medical conditions in a home-based setting
  • Lead interdisciplinary care teams including RNs, APPs, case managers, and virtual support staff
  • Deliver high-quality, value-based care with strong clinical outcomes
  • Improve patient satisfaction and reduce unnecessary hospital stays

Clinical Scope

Typical diagnoses may include:

  • CHF exacerbation
  • COPD exacerbation
  • Pneumonia
  • Cellulitis
  • UTI
  • Post-surgical recovery
  • Other DRG-appropriate conditions per program guidelines
