You will:
  • Conduct comprehensive health assessments and follow-up visits

What We're Looking For
Minimum Qualifications
  • Master of Science in Nursing (MSN)
  • Active, unrestricted Registered Nurse (RN) license
  • Active, unrestricted Nurse Practitioner (NP) license
  • Minimum of one year of Nurse Practitioner clinical experience
  • Strong assessment, diagnostic, and care management skills
  • Valid driver's license and reliable transportation
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Completion of required TB screening

Preferred Qualifications
  • Primary care, family medicine, internal medicine, geriatrics, home-based care, or value-based care experience
  • Experience caring for Medicare and medically complex patient populations
  • Bilingual Spanish-English skills preferred

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.
