BrightStar of Lake County is seeking an experienced and dependable Registered Nurse (RN) for a full -time hybrid role combining skilled field nursing with office-based clinical support.  
 
This role is ideal for an RN who is confident with:

-Skilled nursing assessments and evaluations

-Infusion therapy

-Wound care management

-Care planning and Clinical oversight

Unlike typical PRN home care roles, this position offers consistent week day hours, long-term patient relationships, and the opportunity to grow into clinical leadership or case management.

 
RN Responsibilities
 
Skilled Field Nursing

-Perform nursing assessments, admissions, reassessments, and supervisory visits

-Provide skilled nursing care in patients' homes

-Perform wound care and monitor healing progress

-Manage and support infusion therapy cases

-Identify changes in patient condition and communicate appropriately

-Educate patients and families on medications, treatments and ongoing care needs

-Develop, implement and update individualized care plans

Office / Clinical Support Duties:

-Review clinical documentation for accuracy and compliance

-Process and track physician orders

-Conduct caregiver skills assessments and competency evaluations

-Assist with caregiver on-boarding and orientation 

-Collaborate with scheduling and leadership to ensure continuity of care

-Communicate proactively with the Director of Nursing regarding patient and staff needs
