ChristianaCare's Primary Care at Home is seeking an experienced Registered Nurse II to deliver high-quality, safe, and evidence-based patient care.
This role is responsible for achieving optimal patient outcomes through both independent practice and collaborative teamwork within the clinical setting. The Registered Nurse II will apply the Nursing Process to assess, plan, implement, and evaluate care, contributing as a trusted and integral member of the healthcare team.
Primary Care at Home is in Elkton, Maryland. Primary Care at Home provides in-home medical care for homebound patients who are unable to access traditional healthcare settings. This program utilizes a multidisciplinary, patient-centered approach to provide comprehensive care that addresses patients' medical, emotional, and social needs. Services include chronic disease management, care coordination across home and hospital settings, early identification and intervention for health and social concerns, and referrals to community resources and home-based services such as laboratory testing, imaging, therapy, and hospice care. Additional support includes caregiver assistance, coordination of transportation and meal delivery services, and guidance with insurance and prescription-related issues. The program serves patients throughout New Castle County and Cecil County.
Primary Care at Home is a well-established, team-based primary care practice. The clinical team includes a physician, Nurse Practitioners and an Administrative Assistant. Together, this multidisciplinary team is dedicated to delivering comprehensive, high-quality patient care in a collaborative and efficient healthcare environment.

Work Schedule:
  • Monday through Fridays: 8am to 4:30pm
  • This role involves conducting patient home visits and providing triage support. During the initial six months, the position is primarily office-based while completing orientation and training. Upon successful completion of orientation, there is an opportunity to work remotely on days assigned to triage coverage. Candidates must possess a valid driver's license and maintain active automobile insurance.

Duties / Responsibilities:
As a Registered Nurse II, this role will provide age-appropriate, patient-centered care to pediatric through geriatric populations in both virtual and in-person settings, using strong clinical knowledge to assess and address patient needs.
This role includes delivering services such as wellness visits, care management, point-of-care testing, and triaging patients, while collaborating closely with providers to support examinations, procedures, and efficient clinic operations.
As a Registered Nurse II, will serve as a key liaison among patients, providers, and staff, ensuring clear communication, coordinating workflow, and responding to urgent situations. Will also support staff development through training and mentorship, participates in quality and compliance initiatives, and contributes to process improvement efforts-all while maintaining a strong focus on safety, patient education, and high-quality care delivery.
Education Requirements:
  • A graduate of a state approved or National League of Nursing accredited School of Nursing.
  • Registered Nurse licensure or eligible for licensure in the state Delaware, Maryland or Pennsylvania & New Jersey depending on location of employment.
  • Two years' experience as a Registered Nurse, physician office experience is preferred.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (AND) will be considered with an agreement of completion of BSN in 3 years
  • Basic Life Support (BLS) required

Hourly Pay Range: $38.22 - $61.16
This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
