This full-time position operates within a remote setting, providing flexibility while emphasizing collaboration through telehealth platforms and virtual meetings. The hospice center environment requires a deep understanding of palliative care practices, unique challenges faced by terminally ill patients, and the importance of family involvement in the care process. You will interact closely with an interdisciplinary team, consisting of nurses, physical therapists, and healthcare providers dedicated to delivering comprehensive patient care.

• Act as a liaison between patients, families, and outpatient providers, ensuring effective communication and coordination of care.
• Engage actively with insurance companies for pre-certification and utilization management to facilitate necessary treatments and therapies.
• Collaborate in discharge planning and aftercare strategies tailored to meet individual patient needs, ensuring smooth transitions in care.
• Provide therapeutic meal support, guiding patients on developing a healthy relationship with food while addressing specific dietary concerns related to their conditions.
• Deliver empathetic, safe, and affirming care that respects the unique backgrounds and identities of each client, promoting dignity and self-worth throughout their care journey.

Your primary focus will be on older adults and their families, who require specialized support during end-of-life challenges. The role requires sensitivity and adaptability to meet the distinct needs of this population, addressing not only their mental health needs but also facilitating difficult discussions about end-of-life care and grief management.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of Georgia is mandatory.
• Demonstrated experience working with older adults, particularly in a healthcare or hospice setting.
• Strong clinical and diagnostic skills to effectively assess and develop treatment plans tailored to individual patient needs.
• Excellent communication, interpersonal, and collaborative skills to engage effectively with patients, families, and team members.
• Ability to work independently while also contributing positively to team dynamics.

• Previous experience in hospice or palliative care is highly desirable.
• Familiarity with telehealth platforms and remote care methodologies.
• Expertise in grief counseling and supportive therapy approaches.
