Work EnvironmentThis full-time position operates within a remote setting, providing flexibility while emphasizing collaboration through telehealth platforms and virtual meetings. The hospice center environment requires a deep understanding of palliative care practices, unique challenges faced by terminally ill patients, and the importance of family involvement in the care process. You will interact closely with an interdisciplinary team, consisting of nurses, physical therapists, and healthcare providers dedicated to delivering comprehensive patient care.
Key Responsibilities- Act as a liaison between patients, families, and outpatient providers, ensuring effective communication and coordination of care.
- Engage actively with insurance companies for pre-certification and utilization management to facilitate necessary treatments and therapies.
- Collaborate in discharge planning and aftercare strategies tailored to meet individual patient needs, ensuring smooth transitions in care.
- Provide therapeutic meal support, guiding patients on developing a healthy relationship with food while addressing specific dietary concerns related to their conditions.
- Deliver empathetic, safe, and affirming care that respects the unique backgrounds and identities of each client, promoting dignity and self-worth throughout their care journey.

Qualifications- A Master's degree in Social Work from an accredited institution is required.
