Position Overview:
As a TeleMate Health Nurse, you will play a vital role in delivering patient-centered care. You will play a key role in providing quality patient care through oversight, education, and communication with the patient's existing healthcare team. This position offers flexibility, work-life balance, and the opportunity to make a meaningful impact on patients' lives.


Key Responsibilities

  • Provide oversight on patient's vital signs and taking appropriate action when readings are outside of parameter
  • Being a patient advocate, meeting them where they are in their individual healthcare journey
  • Conduct a thorough, holistic assessment of patients' health conditions and needs
  • Educate patients and caregivers on managing disease processes and medications
  • Educate patients on utilizing RPM technology effectively and efficiently
  • Provide verbal reports to physician offices and help bridge the gap between patient and healthcare provider
  • Effectively provide skilled documentation related to patient communication and education
  • Assess for acute exacerbations when necessary


Qualifications and Skills

  • Graduated from an accredited program
  • Current licensure and accreditation for Licensed Practical Nurse (LPN), or Licensed Vocational Nurse (LVN)
  • Strong communication skills and a commitment to providing compassionate care.
  • Ability to work independently and manage time effectively in a remote environment.
  • Proficiency with electronic health record (EHR) systems and telehealth technologies.
