As an LCSW at our hospice center, your daily responsibilities will include:

• Conducting thorough psychosocial assessments to evaluate the emotional, social, and spiritual needs of patients and their families.
• Developing and implementing individualized treatment plans that align with the patient's goals and values.
• Utilizing evidence-based therapeutic interventions, such as cognitive behavioral therapy (CBT) and mindfulness techniques, to support emotional resilience in patients experiencing anxiety, depression, or grief.
• Documenting all client interactions and maintaining secure, confidential records in our electronic health record (EHR) system.
• Actively collaborating with a multidisciplinary team of healthcare providers, including physicians, nurses, and spiritual care advisors, to ensure comprehensive patient care.
• Providing education and resources to patients and families, guiding them through the complexities of end-of-life care options and bereavement resources.
• Engaging in ongoing professional development by attending workshops, training, and conferences relevant to hospice and palliative care.

This role primarily focuses on supporting patients residing in hospice care, along with their families, navigating the emotional challenges that accompany serious illness and end-of-life transitions. Our clients come from diverse backgrounds and experiences, requiring a sensitive approach tailored to their unique needs as they face life-limiting diagnoses.

To be considered for the LCSW position, candidates must possess:

• A Master's degree in Social Work from an accredited institution.
• A current and valid LCSW license in the state of Tennessee.
• Strong communication, interpersonal, and problem-solving skills.
• A commitment to ethical practices and professional standards in social work.
• A passion for supporting individuals and families facing challenging life circumstances.

While not mandatory, the ideal candidate will have experience in hospice or palliative care settings. Previous work addressing issues related to trauma, grief, or chronic mental health conditions will enhance your effectiveness in this role.
