• Conduct thorough and compassionate mental health assessments, evaluating factors such as personal history, presenting concerns, and risk assessments to identify the unique needs of hospice patients.
• Develop and implement individualized treatment plans that are tailored to the specific emotional and psychological needs of patients, focusing on enhancing their quality of life and easing psychosocial distress.
• Provide evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and supportive counseling, designed to help patients and their families navigate grief and adjust to end-of-life issues.
• Facilitate individual and group therapy sessions, creating a safe space for patients and caregivers to express feelings, share experiences, and develop effective coping strategies.
• Collaborate closely with other healthcare providers, including physicians, nurses, and chaplains, to create a comprehensive care plan that addresses the multidisciplinary needs of each patient.
• Offer crisis intervention services, employing de-escalation techniques and support strategies to assist patients in crisis situations and prevent further emotional distress.
• Educate patients and their families about available community resources, coping strategies, and mental health disorders, empowering them to actively participate in care decisions.
• Maintain accurate and timely documentation of assessments, treatment plans, progress notes, and other necessary records in compliance with all relevant legal and ethical guidelines.
• Participate in virtual team meetings, continuous training, and staff development workshops to foster professional growth and enhance the quality of care provided.
• Stay informed of the latest developments in mental health practices, standards, and regulations pertinent to social work in hospice and palliative care.

In this role, you will primarily serve terminally ill patients and their families, who often face unique psychological challenges during the end-of-life journey. Your ability to provide sensitive, respectful, and informed support will be crucial, as many patients are dealing with complex emotions surrounding grief, loss, and transition.

• Master's degree in Social Work from an accredited institution.
• Active and valid Licensed Clinical Social Worker (LCSW) licensure in the state of Nevada.
• A minimum of two years of post-licensure experience, preferably in mental health or a hospice/palliative care setting.
• Comprehensive knowledge of mental health disorders, treatment modalities, trauma-informed care, and palliative care practices.
• Strong skills in conducting mental health evaluations, diagnosing conditions, and formulating coherent treatment plans.
• Proficiency in individual and group therapy techniques, alongside strong crisis intervention capabilities.
• Excellent communication skills, proficient in building rapport with diverse populations and navigating difficult conversations.
• Ability to maintain professionalism and ethical boundaries while working in high-stress and emotionally charged scenarios.
• Familiarity with electronic health records (EHR) systems to facilitate efficient documentation and care coordination.

• Experience in hospice or palliative care settings.
• Background in working with bereavement support or grief counseling initiatives.
• Training in evidence-based therapeutic models relevant to hospice care.
