Company Description Treasure Coast Hospice is a non-profit community organization that provides compassionate, expert hospice and grief support services to patients and families at the end of life. Founded in 1982, it now serves more than 4,000 patients annually across Martin, St. Lucie, and Okeechobee counties. Through the Treasure Coast Hospice Foundation and strong community support, the organization offers comprehensive programs, including three inpatient units, a specialized pediatric program (Little Treasures), and therapies such as music, massage, virtual reality, and bereavement services. Treasure Coast Hospice also supports children through Camp Good Grief, helping those who have experienced the loss of a loved one. The organization is recognized as a certified Great Place to Work and holds The Joint Commission’s Gold Seal of Approval® for hospice accreditation.
Role Description The Social Worker is a full-time, on-site role based in Stuart, FL, providing psychosocial support to hospice patients, caregivers, and families. Day-to-day responsibilities include conducting psychosocial assessments, developing care plans, facilitating individual and family counseling, and coordinating community resources to address emotional, social, and practical needs. The Social Worker collaborates closely with an interdisciplinary team including nurses, physicians, chaplains, and volunteers to ensure holistic, patient- and family-centered care. Additional duties include crisis intervention, advocacy, participation in bereavement programs, documentation in accordance with regulatory and organizational standards, and support for educational and outreach activities.
