Job description

Home Health Nurse (LPN/RN)

Whitmore Lake, Michigan, United States

Care in the Heart of Whitmore Lake: Join a Dedicated Home Team

Imagine a nursing shift where the "hustle" of a hospital is replaced by the quiet rhythm of a lakeside home. We are looking for a compassionate LPN or RN to support our client, a man in his early 40s who lives with his mother and sister. While he is non-verbal and paralyzed, he has a strong presence and a family that is deeply committed to his comfort.

This isn't just a clinical role; it's an opportunity to provide the kind of dedicated, one-on-one care that allows a son and brother to thrive in the place he loves most.

A Day in the Life

Your day will include:

  • Respiratory Support: Keeping his lungs clear using a nebulizer and a cough assist machine three times a day.
  • Nutrition: Managing his feeding tube and ensuring his daily intake is on track.
  • Safe Movement: Using a ceiling lift for transfers—allowing you to move the client safely and comfortably without physical strain.
  • Physical Therapy: Coordinating with the family to get him ready for his weekly PT appointments.
Who We Are

Centria Healthcare is committed to delivering exceptional, personalized care to clients of all ages. Our mission is simple: do whatever it takes to provide superior home healthcare services to individuals with medically complex conditions by putting our clients' needs at the center of everything we do.

We specialize in:

  • Pediatric Care: Children with tracheostomies, ventilators, seizure disorders, and other complex medical needs. Adult Care: Individuals recovering from motor vehicle accidents facing injuries such as spinal cord damage, TBI, orthopedic injuries, and more.
Why Join Centria?
  • Flexible Scheduling: Full-time, part-time, and per diem options to fit your life.
  • Competitive Pay: Weekly pay.
  • Meaningful Work: Build long-term relationships with patients and their families and truly make a difference in their life.
  • Supportive Training/Resources:
    • Virtual Reality + Hands-on Simulation
    • Shadowing shifts
    • Clinical Procedure Manual embedded in EMR
    • 24/7 Clinical On-Call Support
  • Professional Growth:
    • Shared Governance Committee
    • Clinical Quality Metrics Measured Monthly
    • DAISY Recognition
    • Opportunities for advancement within a growing company
  • Virtual Onboarding: Making your onboarding process a breeze, along with a virtual orientation
  • Comprehensive Benefits (for eligible staff): Health insurance, paid sick-time, and more
Responsibilities
  • Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)
  • Monitor and document vitals, symptoms, and changes in condition
  • Assist with ADLs such as repositioning, hygiene, mobility, etc.
  • Support patient well-being with compassionate and attentive care
  • Collaborate with families, caregivers, and the healthcare team
Qualifications
  • Active RN or LPN license in the state of Michigan
  • Experience in home health, private duty, or long-term care preferred
  • Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)
  • Strong communication skills and a passion for patient-centered care
