Your responsibilities include: Conducting patient visits to assess nursing needsCompleting initial evaluation visits and developing individualized plans of carePerforming ongoing assessments and revising plans as needed under physician directionProviding skilled nursing treatments and educating patients and caregivers on disease processes, self-care, and preventionInitiating preventive and rehabilitative nursing procedures as orderedPreparing clinical and progress notes and coordinating services with other care team membersCommunicating directly with physicians and staff to ensure continuity of careRe-evaluating patient needs and assisting with discharge planningSupervising LPNs, vocational nurses, and home health aides when appropriatePracticing infection control and universal precautionsReporting suspected abuse according to state and federal guidelinesAssisting with office tasks and new-nurse orientation when neededYou take pride in providing high-quality care that truly impacts patients' lives.
