Responsibilities
  • Provide high-quality 1:1 nursing care in patients' homes
  • Assess patient needs and implement individualized care plans
  • Deliver treatments and skilled nursing services within the RN scope of practice
  • Monitor patient progress and communicate changes to the care team
  • Maintain accurate, timely, and compliant clinical documentation
  • Follow all company policies, clinical protocols, and regulatory requirements
  • Ensure HIPAA compliance and protect patient confidentiality
Required SkillsRequired
  • Active, unrestricted Registered Nurse (RN) license in Illinois or a Compact Multi-State RN License valid in Illinois
  • Minimum of 1 year of nursing experience
  • Drivers License, reliable transportation and auto insurance
  • Ability to pass background screening
  • Knowledge of state and federal healthcare regulations
  • Strong clinical, communication, and critical thinking skills
Preferred
  • 3-5 years of clinical nursing experience
  • Previous experience in home health, primary care, or a healthcare facility
  • Bachelor's Degree in Nursing (BSN)
  • Graduate of an accredited nursing program
  Physical Requirements
  • Ability to stand, walk, bend, squat, kneel, reach, push, pull, and lift during patient care activities
  • Ability to lift up to 50 pounds with or without reasonable accommodation
  • Visual and hearing acuity sufficient to safely perform nursing duties
