Job description

  • Manage referrals and schedule patients in a timely manner
  • Perform patient evaluations
  • Document communication notes for evaluations not completed within specified timeframes
  • Supplement the virtual caseload with in-person house calls, SLC visits, and other patient-care tasks as needed
  • Perform and document virtual patient screenings
  • Coordinate with regional clinicians on patient transitions, care coordination, and evaluation shadowing
  • Apply virtual program components, including tests and measures, interventions, and patient/outcome tracking
  • Participate in virtual care marketing
  • Participate in telehealth teaching and training for field clinicians
  • Attend recurring meetings with leadership and Virtual Care Team members
  • Participate in practice initiatives, sales, and marketing efforts
  • Meet contract quota expectations bi-weekly
  • Maintain KPI standards monitored by leadership
  • Ensure compliance with FOX, federal, state, and payer regulations and policies for telehealth outpatient rehabilitation services
  • Maintain professional attire and a neat appearance
  • Communicate effectively with team members and seniors
Requirements
  • Must be licensed as a Speech Language Pathologist (SLP)
  • SLP license in Virginia required
  • ASHA Certification required
  • Must meet all educational/credential requirements of a Speech Language Pathologist
  • Willingness to obtain and maintain licensure in other states as determined by leadership
  • Experience treating the older adult population
  • 1 year of telehealth experience
  • Willingness to provide in-person house calls and/or SLC visits as needed
  • Ideal candidate would live in an SLP compact state
