Position Summary:

VitalCare is seeking licensed and credentialed Nurse Practitioners (NPs), Physician Assistants (PAs), Doctors of Osteopathic Medicine (DOs), and Medical Doctors (MDs) to join our growing telehealth team. As a Telehealth Provider, you will deliver exceptional virtual care, diagnose and treat a variety of conditions, and ensure patients receive the highest standard of care.


Key Responsibilities:


  • Conduct telehealth visits for patients in residential facilities (primarily SNFs).
  • Perform virtual assessments, diagnose conditions, and develop treatment plans.
  • Prescribe medications and provide appropriate follow-up care.
  • Collaborate with on-site nurses and staff to optimize patient outcomes.
  • Maintain accurate and timely documentation in the electronic medical record (EMR) with AI support.
  • Provide patient education and support during telehealth sessions.


Qualifications:


  • Active licensure as an NP, PA, DO, or MD in any of these states: AZ, CA, CO, FL, ID, MA, ME, MN, MO, NC, NH, NV, OH, OK, OR, KY, RI, TX, WA, WY.
  • SNF experience preferred.
  • Previous telehealth experience is a plus but not required.
  • Strong communication and technology skills.
  • Ability to work independently while collaborating with the healthcare team.
  • Responsive.
  • Must be DEA-registered in the states to be practiced in.
