ED Licensed Masters Social Worker

As our part time Care Coordination Licensed Masters Social Worker, you will be instrumental in providing comprehensive social work assessments and interventions for emergency department patients. This includes crisis intervention, high-risk screening, and patient/family support to facilitate safe discharge planning and ensure continuity of care. You will be a key advocate for patients and families, navigating complex healthcare needs, and connecting them with vital community resources.

Every day you will perform psychosocial assessments, provide counseling support, and identify needs related to abuse, neglect, trafficking, and social determinants of health. You'll facilitate patient/family care conferences, manage post-acute placements for complex discharges, and educate patients and families on available community resources, including mental health services, palliative care, and hospice. Your role will involve maintaining up-to-date resource lists, contributing to an interdisciplinary team, and upholding ethical standards while adhering to federal and state regulations.

To be successful in this role, you will demonstrate excellent customer service, strong interpersonal and written communication skills, and proven analytical and problem-solving abilities. Your capacity to manage multiple tasks, work independently with high detail orientation, and thrive in a fast-paced environment while collaborating effectively with interdisciplinary teams is crucial for success.

Required:

  • 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
  • LMSW license from the AZ Board of Behavioral Health Examiners (MSW: AZ)
  • Master's Degree from a School of Social work accredited by the Council of Social Work Education
  • Ability to work independently as needed with a high degree of detail orientation.
  • Ability to work efficiently in a fast-paced environment with changing priorities.
  • Ability to work collaboratively with an interdisciplinary care team.

Preferred:

  • Minimum 3-Year Post-MSW healthcare experience
