This position offers a competitive annual salary ranging from $75,000 to $100,000, along with a sign-on bonus. As an LCSW, you will play a vital role in our patient's journey toward recovery, addressing their emotional, social, and practical needs in a supportive and rehabilitative environment.

Key Responsibilities

  • Provide therapeutic counseling and emotional support to patients and their families, addressing issues related to adjustment, grief, and emotional welfare during their rehabilitation journey.
  • Collaborate effectively with a diverse rehabilitation team to develop and implement individualized treatment plans that consider the unique psychosocial needs of each patient.
  • Assist patients in navigating community resources, including support groups, vocational rehabilitation, and housing assistance, ensuring that they have access to the support they need to thrive.
  • Educate patients and their families on community resources and post-rehabilitation options, empowering them to make informed decisions about their care.
  • Play an integral role in discharge planning by facilitating smooth transitions to home or alternative care settings, working closely with multidisciplinary teams.
Patient Population / Client Focus

Qualifications
