• Assist patients in navigating community resources, including support groups, vocational rehabilitation, and housing assistance, ensuring that they have access to the support they need to thrive.
• Educate patients and their families on community resources and post-rehabilitation options, empowering them to make informed decisions about their care.
• Play an integral role in discharge planning by facilitating smooth transitions to home or alternative care settings, working closely with multidisciplinary teams.

Our LCSWs work with a diverse client base, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. • A Master’s degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is necessary for this position.
• Excellent communication and interpersonal skills are essential for effective collaboration with patients, families, and multidisciplinary teams.

• Previous experience in a rehabilitation or medical setting is highly preferred, with a focus on patient-centered care.
