Job description

Position Purpose:

The purpose of the Clinic Nurse-RN position is to deliver high-quality, patient-centered care by assessing, planning, implementing, and evaluating nursing interventions in collaboration with interdisciplinary healthcare teams. The Clinic Nurse plays a critical role in promoting health, managing chronic and acute conditions, ensuring patient safety, and enhancing overall patient outcomes through evidence-based practices, compassionate care, and adherence to organizational and regulatory standards. As a Clinic Nurse at JWCH, you will have the opportunity to help improve the lives and health status of the underserved segments of the population of the greater Los Angeles area through the direct provision of coordinated health care, health education, health services, and research. 

Principal Responsibilities:

  1. Collaborates as a core member of the interdisciplinary team to deliver comprehensive medical care to residents.
  2. Conducts triage for all walk-in patients, performs thorough patient assessments, creates care plans, and implements evidence-based nursing interventions.
  3. Administers medications, treatments, and procedures in alignment with established protocols and nursing standards, including utilizing standing orders.
  4. Monitors and manages the physical and mental health needs of residents, demonstrating sensitivity to their unique challenges.
  5. Engages in health education initiatives and promotes preventive care to empower residents in taking control of their well-being.
  6. Provides compassionate and culturally sensitive support to residents facing behavioral and substance abuse issues, adhering to the Harm Reduction Model.
  7. Maintains accurate and up-to-date documentation of all patient interactions, care provided, and medication/medical supply inventory.
  8. Completes TB screenings/readings and ensures proper documentation.
  9. Administers oral, IM, SC, and SL medications as needed.
  10. Assists medical providers with procedures as necessary.
  11. Performs I.V. placement and monitoring, including the infusion of I.V. fluids as ordered by the provider.
  12. Addresses patient complaints and follows appropriate procedures for resolution.
  13. Provides patient education and teaching, including scheduling hospital or specialist appointments if necessary.
  14. Assigns daily nursing duties and identifies and resolves patient flow deficiencies.
  15. Organizes and maintains dispensary medication inventory, including removing expired medications and preparing them for proper disposal.
  16. Prepares and dispenses all medications as ordered by the provider, ensuring proper labeling of dispensed medications.
  17. Maintains and documents the emergency kit (E-kit) as required.
  18. Places orders for medical supplies as needed.
  19. Collaborates with the Clinic Administrator to support continuous improvement of systematic functions.
  20. Develops and monitors communication networks between the Medical Director, back office staff, social workers, providers, and external staff to improve continuity of care and prevent conflicting information regarding patients' medical plans of care.
  21. Educates, trains, and motivates back office staff to reach their highest potential by identifying areas for individual educational and training needs.
  22. Monitors and ensures adherence to the Nursing Training Manual and agency policies and procedures.
  23. Ensures exam rooms are clean, stocked, and maintained according to standards.
  24. Completes and reviews logs (e.g., refrigerator, oxygen, etc.) for accuracy and compliance.
  25. Ensures proper calibration, maintenance, and yearly documentation of equipment.
  26. Reviews and monitors the Facility Site Review (FSR) tool in collaboration with the Clinic Supervisor.
  27. Monitors and completes HEDIS measures, including identifying and following up on pending measures to ensure completion.
  28. Closes patient encounters accurately and in a timely manner.
  29. Participates in e-kit maintenance, ensuring proper functionality and readiness for emergencies.
  30. Participate in JWCH’s customer service training called AIDET on an annual basis.
    1. Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions.
    2. Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes.
  31. Performs other duties as required by the program and/or clinic.

Requirements:

  1. Possess an active and unrestricted Registered Nurse (RN) license in the state of California. 
  2. Current CPR card by American Heart Association 
  3. Minimum of 1–2 years of clinical nursing experience; at least one-year experience in an ambulatory care setting.
  4. Proficiency in electronic health record (EHR) systems and familiarity with medical equipment.
  5. Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare teams.
  6. Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  7. Possess a valid California Driver's license and able to travel to different sites.
  8. Bilingual (Spanish/English) preferred.
  9. Problem-Solving Abilities: Critical thinking and decision-making skills for assessing and addressing patient needs.
  10. Physical Stamina: Ability to stand, walk, and lift patients or medical equipment as necessary.
