Community Health Care Coach

Salary: $41,350.40

Job Overview: Elder Options is seeking a Community Health Care Coach, to provide community health education, follow up support and coaching to patients. Home visits are required and there is travel reimbursement. Remote work is limited.

Key Responsibilities: Services include Care Transition services supporting hospital to home transitions, public health efforts and/or other community-based health and wellness services. The interventions are geared toward patient self-management of chronic diseases via teaching/modeling, self-advocacy, patient advocacy and education with the purpose of reducing re-hospitalizations, providing individualized assessments, care planning, referrals and supports to adults with health and well-being goals to support their independence in the community.

Minimum Qualifications: Bachelor’s degree (B.A./B.S.) from four-year college or university or equivalent experience in providing human services.

Required Skills and Abilities: Successful completion in the Take Charge Care Transition Model is required for maintaining position. Training will be provided.

Preferred Qualifications: Experience working with older and/or disabled adults and Spanish-speaking preferred. Located in Lake or Marion County.
