Job Description
This role is fully remote and must be located within one of the contiguous 50 states. You must be able and willing to work 3x12s or 4x10s as long as Saturday and Sunday are covered.
A bit about this role:
The Digital Care Program and the Responsive Care Department are redefining how we connect with patients by delivering exceptional clinical care entirely through chat and other digital channels. Our mission is to meet patients where they are - online, in real time, and with the same compassion, expertise, and partnership they would expect in person.
As an Advanced Practice Provider (NP or PA) focused on Digital Care, you will be a cornerstone of this mission. You'll provide holistic, empathetic care to patients across multiple geographies, leveraging your clinical expertise and keen attention to detail to ensure their needs are understood and addressed. Digital Care APPs are not just providers; they are trusted partners, guiding patients through symptom management, chronic disease care, medication review, and coordination across their broader healthcare teams within and outside of Devoted - all within a digital-first experience. This role requires the agility to respond to real-time patient demand, manage evolving digital workflows, and flex across Responsive Care programs while maintaining exceptional clinical quality, safety, and patient experience.

Required skills and experience:
  • APP with three or more years working with experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care and/or urgent care.
  • Ability to review AI-generated responses for clinical accuracy and safety.

Licensure and certification
  • For Nurse Practitioners: active and in-good-standing RN and APRN licenses.
  • For Physician Assistants: an active and in-good-standing Physician Assistant license.
  • Role licensure and certification in good standing is required and the ability to get licensed in requested states within 90 days of hire date, starting at offer letter signing. You will be required to get licensed in additional states as needed. We believe in making it easy for you to focus on patient care. Devoted Health will cover all costs and provide full administrative support for obtaining and maintaining licenses in all required states.

Desired skills and experience:
  • Prior virtual care or telehealth experience preferred, with passion for engaging patients digitally.
  • Experience working in real-time, queue-based, or high-volume telehealth clinical environments.
  • Experience collaborating with interdisciplinary care teams and coordinating care remotely.
  • Comfort managing complex or vulnerable patient populations in a digital-first setting.
  • Ability to thrive in a fast-paced, rapidly evolving environment with flexibility and curiosity.

Responsibilities and Impact will include:
  • Deliver exceptional, empathetic clinical care through real-time and asynchronous chat and other responsive care channels, supporting both the digital patient population and evolving queue-based needs across Responsive Care Department and the broader organization.
  • Manage multiple concurrent and asynchronous patient conversations while maintaining clinical accuracy, timely documentation, and reliable follow-through.
  • Guide and oversee patients' medical management using a holistic framework (including the 5M approach: Mind, Mobility, Medications, Multicomplexity, Matters Most).
  • Collaborate closely with patients' primary care providers, specialists, and other teams such as pharmacy and specialty care to ensure continuity and comprehensiveness of care.
  • Support patients' care coordination with their family members and caregivers when appropriate, ensuring alignment with the patient's priorities and goals.
  • Provide thorough assessment, clinical guidance, and follow-up in response to patient-reported symptoms, medication questions, lab results, or new health concerns.
  • Recognize when a patient's needs cannot be safely or effectively addressed through chat and facilitate escalation to telephone, video, in-person, urgent, or emergency care as appropriate.
  • Maintain ownership of patient needs through resolution or a clear, documented handoff.
  • Maintain meticulous, accurate, and timely digital documentation in the electronic health record (EHR) in compliance with legal, ethical, and telehealth regulations.
  • Continuously identify opportunities to improve patient experience and outcomes, leveraging digital workflows and proactive communication.
  • Contribute to the improvement of digital workflows, standardized responses, escalation pathways, and quality standards.
  • Occasionally support broader Devoted Medical service lines in line with program growth and team needs.
  • Provide flexible clinical coverage across Responsive Care Department queues and on-call services as patient demand and operational priorities evolve.
  • Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.

Salary Range: $120K-155K base range plus performance based bonus paid out quarterly or annually ($10K-$20K) for a total comp range of $130K-170K

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
