Job description

Clinical Team Lead

 In this key leadership role, you'll help guide a multidisciplinary team of clinicians, support high-quality patient care, and make a meaningful impact on the lives of patients and families in the comfort of their homes.

What You'll Do

As the Clinical Team Lead, you will:

  • Provide clinical leadership and support to nurses, aides, therapists, and social workers
  • Coordinate patient care, clinician assignments, and referrals
  • Oversee care plans to ensure quality outcomes and regulatory compliance
  • Review clinical documentation and support timely billing and reimbursement processes
  • Monitor high-risk patients and collaborate with clinicians to reduce hospitalizations
  • Conduct chart audits and participate in quality improvement initiatives
  • Serve as a clinical resource and mentor for field staff
  • Support staff onboarding, education, and competency development
  • Step into leadership responsibilities in the absence of the Clinical Director/Manager
What We're Looking For
  • Active Ohio RN or LPN license
  • Minimum 3 years of clinical nursing experience
  • At least 1 year of home health experience required
  • Previous supervisory or leadership experience preferred
  • Strong communication, critical thinking, and organizational skills
  • Valid driver's license and reliable transportation
