Position Summary

Unity Health Care is seeking a Virtual Care Team (VCT) Medical Assistant to join our innovative, grant-funded Virtual Care Team dedicated to transforming healthcare delivery through technology. This position plays a vital role in supporting patients remotely while collaborating with providers, nurses, and population health teams to improve patient engagement, care coordination, and health outcomes.

This is a hybrid position offering scheduling flexibility; however, the successful candidate must remain highly accountable to team priorities, patient outcomes, and effective communication in a remote and asynchronous work environment. Training will be conducted on-site for up to 90 days, and candidates must reside within commuting distance of Washington, DC.

Key Responsibilities
  • Provide patient-centered support through virtual and telehealth services.
  • Obtain and document vital signs, medical histories, chief complaints, and other clinical information in the Electronic Health Record (EHR).
  • Prepare patients and providers for virtual visits and assist providers during patient encounters as needed.
  • Utilize motivational interviewing techniques to engage patients and promote healthier behaviors.
  • Assist with medication refills, referrals, appointment scheduling, and follow-up care.
  • Support chronic disease management and preventive care initiatives by identifying and closing gaps in care.
  • Coordinate care between patients, providers, specialists, hospitals, pharmacies, community resources, and managed care organizations.
  • Monitor and track high-risk patient populations, hospital discharges, emergency department visits, and missed appointments.
  • Assist patients in overcoming barriers to care, including transportation, insurance, financial assistance, language services, and community resources.
  • Participate in interdisciplinary care team meetings and case conferences.
  • Maintain accurate, timely documentation while ensuring compliance with HIPAA and Unity Health Care policies.
  • Deliver exceptional customer service while supporting Unity's mission of patient-centered care.
Minimum Qualifications
  • High school diploma or GED required.
  • Clinical Medical Assistant certification
  • Current Basic Life Support (BLS) and AED CPR certification.
  • Reliable high-speed Wi-Fi service.
  • Dedicated home workspace suitable for conducting telemedicine visits.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary virtual care team.
