Job description

Certified Nursing Assistant (CNA)

Daymark Health is a value-based oncology company redefining the cancer care experience for patients, providers, and health plans. Daymark's comprehensive, personalized cancer care platform empowers patients with dedicated care navigation, symptom-focused support, behavioral health care, and social resources. Combined with evidence-based health interventions and a hybrid in-person + virtual care model, Daymark is improving the overall cancer experience for patients, providers, and health plans โ€“ and setting a new standard in cancer care.

Daymark's groundbreaking approach is led by CEO Dr. Justin Bekelman, a pioneer in transforming cancer care, alongside some of the nation's foremost leaders in oncology and value-based care. Daymark Health is backed by Maverick Ventures, Yosemite, Oncology Ventures, Healthier Capital, Blue Venture Fund, and Healthcare Foundry.

Be cautious of recruitment fraud, and always confirm that communications are coming from an official Daymark Health email.

About The Role

Daymark is looking for candidates with a Certified Nursing Assistant background for our "Health Partner" role within our Care Team. Health Partners are core members of the Daymark care team, supporting integrated, whole-person care for our patients. In this role, you will coordinate social and logistical aspects of care, spending time connecting with patients and caregivers by phone, video, and occasionally in the home. You will help patients navigate community-based and social services, ensure follow-up appointments are scheduled, and support seamless coordination between Daymark and external providers.

What You'll Do
  • Conduct structured social needs assessments (SDOH) and connect patients with community resources, which may include transportation, financial assistance, food, medical equipment, or emotional support programs.
  • Support development of patients' care plans and participate in case conferences to ensure quality care delivery.
  • Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
  • Coordinate and schedule care for patients, including scheduling follow-up visits and facilitating referrals.
  • Own structured chart preparation to ensure providers are visit-ready, including accurate contact, insurance, and care team information.
  • Track completion of our Symptom Survey, complete all appropriate follow ups, and escalate clinical concerns as needed.
  • Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.
What We Are Looking For
  • Experience: You have at least 3 years of experience as a Medical Assistant, CNA, EMT, or in a similar clinical or care coordination setting. Oncology experience is a bonus. You're comfortable driving to visit patients in the field.
  • Comfort with Technology: You're comfortable using Slack, Zoom, the Google Suite, and EHR systems to ensure accurate appointment scheduling and management and support patients virtually.
  • Bias to Action: You don't wait for someone to tell you what to do. You know how to identify and prioritize your initiative's needs and you do what it takes to ensure that urgent and important needs are acted on immediately.
  • Comfort with Change and Ambiguity: You may not always have direction in your work, and that's OK, because you're comfortable figuring out what needs to be done and a path forward.
  • Strong Relational Skills: You have an uncanny ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Organized & Communicative: You are incredibly organized and sweat the details. You keep your team in the loop with what you're working on and the challenges you're facing and are proficient in collecting patient clinical and demographic data and documenting appropriately in a timely manner.
  • Open to Working from Daymark's Office: You're comfortable working out of our office in Providence, Rhode Island 5 days of the week, and visiting patient's homes whenever needed.
  • Bilingual: is a plus but is not required.

Daymark Health is an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.

All Daymark Health patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. Candidates without these at the time of hire should coordinate with their recruiter and hiring manager to ensure they meet the requirements by their start date.

All roles that require in-home visits to patients must provide proof of a valid driver's license.

Compensation Range: $50,000 - $60,000 USD
