• Deliver Exceptional Patient Care
  • Provide direct physical therapy services under the supervision of a licensed Physical Therapist.
  • Implement personalized care plans to alleviate pain, enhance mobility, and restore function.
  • Assist patients with therapeutic exercises, gait training, balance, and daily activities.
  • Support patients in transfers, strengthening exercises, and daily living tasks.
  • Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as needed.
  • Educate patients and caregivers on exercises, safety measures, and the use of assistive devices.
• Monitor Progress & Communicate
  • Observe and report on patient progress, treatment responses, and any changes in condition.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Effectively communicate with the Physical Therapist and interdisciplinary care team.
  • Engage in case conferences and care coordination activities.
• Collaborate & Contribute
  • Work closely with nurses, therapists, and other healthcare professionals to ensure cohesive care.
  • Participate in team meetings, training sessions, and ongoing professional development.
  • Adhere to clinical standards and contribute to quality improvement initiatives.

• A strong patient-first attitude with a dedication to rehabilitation.

• Excellent communication and time management abilities.

• Graduation from an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as mandated by state regulations.
• Preferred experience of 1+ year (home health experience is advantageous).
• Current CPR certification.
• A valid driver’s license, dependable transportation, and auto insurance.
