Choice Health at Home is seeking a compassionate and skilled Physical Therapist to provide individualized care that helps patients regain mobility, improve function, manage pain, and maintain independence in the comfort of their own homes.
Responsibilities
  • Perform comprehensive patient evaluations and develop individualized treatment plans.
  • Implement therapeutic interventions to restore function, improve mobility, and reduce pain.
  • Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
  • Collaborate with physicians, nurses, caregivers, and interdisciplinary team members.
  • Educate patients and families on treatment goals and home exercise programs.
  • Maintain timely, accurate documentation and comply with agency policies and regulatory requirements.
  • Follow infection control and patient safety standards.
Qualifications
  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the state of practice.
  • CPR certification required.
  • Valid driver's license and auto insurance.
  • Home Health experience preferred.
  • HomeCare HomeBase experience preferred.
