Work Environment

This position is primarily onsite, working within a home care setting located in the vibrant area of Punta Gorda, Florida. Key Responsibilities
  • Conduct comprehensive evaluations of patients in their home environments to develop tailored treatment plans that address specific rehabilitation needs.
  • Utilize a diverse range of therapeutic modalities, including manual therapy techniques, exercise programming, and specialized equipment, to alleviate pain and improve mobility.
  • Document patient progress meticulously using EMR software, especially Web PT, to ensure accurate tracking and evaluation of therapeutic interventions.
  • Regularly reassess patient needs, updating treatment plans and goals as required to optimize rehabilitation outcomes throughout the therapy process.
  • Educate patients, families, and caregivers on the use of assistive devices and physical therapy procedures to empower them in managing their own care.
  • Engage in continuous professional development by participating in educational workshops and staying current with the latest research and physical therapy practices.
Patient Population / Client Focus

In this role, you will work with a diverse patient population, including children, adults, and seniors recovering from injuries, surgeries, or chronic conditions. The variety of diagnoses you will encounter requires adaptability and a deep understanding of individual patient needs for effective care delivery. Your therapeutic interventions will directly influence the independence and quality of life of the patients you serve.

Qualifications
  • Must hold an active Physical Therapist license in Florida, with a clean record free from sanctions or disciplinary actions.
  • Completion of an accredited Doctorate in Physical Therapy (DPT) program.
  • Possess a strong foundational knowledge of manual therapy techniques and exercise prescription.
  • Demonstrated proficiency in utilizing electronic medical record systems, specifically Web PT, for comprehensive documentation.
  • Up-to-date knowledge of evidence-based practices in physical therapy, ensuring compliance with current standards of care.
Preferred Experience
  • Previous experience in home care or outpatient settings, particularly with diverse patient populations.
  • Specialization in specific physical therapy domains, such as geriatrics, pediatrics, or orthopedics.
  • Experience in mentoring or leading therapy teams is a plus.
Compensation and Benefits
