Offering skilled nursing and therapy services, Bethany Village Home Health Care makes it easier for seniors to recover at home and live independently. Caregivers can feel peace of mind knowing their loved one is being carefully and faithfully served
This will be a full time or part time day shift- flexible hours/ schedule
Full Time needs are all over the territory
Part time needs focus mainly on Centerville, Kettering, and Beavercreek area.
Pay Range: 42.50-48.80 (Base) pay per visit point system/ increases based on visit type
Position Summary:
Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all client programs. Administers physical therapy in compliance with Federal, State, local, and Bethany Village Home Health Care standards, policies, and guidelines.
Job duties:
  • Administers physical therapy in compliance with Federal, State, local and Bethany Village Home Health Care standards, policies, and guidelines ensuring quality care and service to clients.
  • Interacts with health professionals, clients, families, and support groups to ensure maximum rehabilitation, while serving as a client advocate at all times.
  • Evaluates clients by administering muscle, neurological, gait, and functional tests.
  • Establishes treatment goals based on the evaluation of the client in conjunction with the client's needs.
  • Develops treatment programs and provides ongoing assessments and modifications as changes in the client's condition occur.
  • Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records in accordance with agency policy and that support charges for services provided.
  • Promptly reports incidents, accidents, changes of condition, abuse, concerns to the assigned staff nurse or the Director of Bethany Village Home Health Care .
  • Complies with OSHA standards and ensures the safety of the client in their care at all times.

Qualifications:
  • Current Ohio Licensed Physical Therapist
  • CPR certification.
  • Valid drivers' license and a safe driving record.
