Job Description
A bit about this role:
The Care Coordinator has an important role supporting TOC Clinical Operations as we ensure our patients have appropriate post discharge planning and follow up in place. The Care Coordinator will support the team with patient outreach and coordination while the patient is inpatient and post discharge.
Your Responsibilities and Impact will include:
  • Reaching out to patients while inpatient to introduce TOC program and communicate post discharge follow up from TOC clinicians
  • Scheduling post discharge follow ups (PCP/Specialty)
  • Reaching out to hospitals and provider offices to obtain clinical documentation
  • Submitting Home Care and DME orders as well as vendor follow up
  • Monitoring return fax queue and researching provider profile for updated contact and fax information
  • Provide additional operational support for assigned team and manager as designated
  • Ensure patients are prepared for telehealth visits by assisting with technology prep
  • Collaborate internally in clinical rounds to discuss patients at risk for readmission
  • Coordinate lab testing and imaging, and tracking results to ensure that they are reviewed by the ordering clinician

*This role does not include clinical decision-making or any activity requiring licensure, and that all clinical tasks are delegated by, and performed under the supervision of, licensed professionals. Clinical Coordinators should direct clinical concerns raised by patients to the appropriately licensed team.]
Work Schedule
  • Schedules Available: Monday - Friday 9a-5:30p ET, 9:30a-6p ET, 10a-6:30p ET, 10:30a-7p ET
  • *Please note, schedules will be discussed during the interview process based on availability. Schedules are selected during time of offer on a first-come, first-service basis.

Required skills and experience:
  • 2+ years of patient facing and care coordination experience within healthcare, managed care, home health, and/or community health setting.
  • Experience working in a telemedicine environment.
  • Fully bilingual (English/Haitian Creole).

Desired skills and experience:
  • Managed Care / Health Plan experience
  • MA Certification (desired), expired MA certifications are OK! [Verification: A copy of your active or expired certification will be required. No Certification: If you have significant MA experience/background, we may request copies of your MA school diploma and transcripts.]

  • Highly organized and adaptable, able to manage a large member panel amidst frequent changes

Salary Range: $23 -$27 / hour
The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
