What You Will Do:
  • Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC.
  • Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care.
  • Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisions
  • Submits clear and appropriate documentation in a timely manner and documents patient education.
  • Communicates effectively to physicians, supervisors, and co-workers in a timely manner.
  • Promotes continuity of care with appropriate admissions, transfers, and charges.
  • Other duties as assigned.

Skills:
  • Effective communication skills (verbal and written)
  • Experience with Electronic Medical Records (EMR) software is preferred
  • Time management and Organizational skills
  • Demonstrated ability to work autonomously as a registered nurse
  • Excellent planning, organizing and communicating.

What you will need:
  • Graduate of an accredited school of professional nursing, BSN preferred, currently licensed as an RN through the Ohio State Board of Nursing.
  • Prior Nursing experience in Hospice or Home Health setting preferred. At least one year's medical/surgical nursing experience preferred.
  • BLS/CPR Certification required
  • Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Independent decision-making skills and abilities to render nursing care in an independent setting.
  • Ability to rotate "On Call" after hours, weekends and holidays.
