We are currently seeking a Nurse Assessment Coordinator.

Job Responsibilities and Duties:

The Nurse Assessment Coordinator is responsible for overseeing the assessment and care planning process for residents in accordance with federal and state guidelines. This includes the coordination and completion of the Minimum Data Set (MDS) assessments, care plans, and related documentation to ensure accurate reimbursement and high-quality care.

  • Coordinate and complete accurate and timely MDS assessments and care plans for all residents.

  • Ensure compliance with all federal, state, and company regulations and guidelines.

  • Work collaboratively with interdisciplinary teams to gather information and develop individualized care plans.

  • Monitor and analyze data for trends, accuracy, and quality assurance.

  • Participate in care plan meetings, audits, and surveys as needed.

  • Educate staff on documentation best practices and changes to MDS guidelines.
