About Us:
Upperline Health is redefining primary care for medically complex patients through a value-based, outcomes-driven care model.
We are seeking a Primary Care Physician (MD/DO) for our mobile physician group who wants to practice thoughtful, value-based care medicine. This group visits Assisted Living Facilities throughout Central Florida.
This is an opportunity to provide professional physician services in an integrated specialty and primary care organization, focused on chronic disease management, prevention, and long-term patient outcomes.
About the Role:
  • Monday - Friday, 8am - 5pm, no nights and weekends.
  • Shared provider call
  • The ability to impact long-term outcomes, not just treating symptoms
  • As Upperline continues to expand its innovative approach to complex chronic care, this role offers career growth, leadership development and partnership opportunities.

Upperline Health providers coordinate patients' care among a team of specialists - Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients' immediate and long-term health needs. We put patients at the center of value-based care. This is a unique role designed for physicians who want more than traditional primary care.
Compensation & Benefits
  • Market leading pay up to $300K annually
  • $50K Starting bonus
  • Competitive benefits - comprehensive health, dental, vision, 401K (matching), PTO and parental leave
  • Medical malpractice, licensure and board dues paid annually
  • CME expense allotment

What you'll do:
  • Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
  • Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
  • Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
  • Order, review, and interpret laboratory tests and diagnostic imaging.
  • Coordinate and manage referrals to specialists and follow up on patient outcomes.
  • Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
  • Maintain timely, accurate clinical documentation in the electronic health record.
  • Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications
  • Medical Doctor or Doctor of Osteopathic Medicine required
  • Successful completion of accredited Internal Medicine or Family Medicine residency program required
  • Board certification or board eligibility required
  • Medical license required - active license preferred or the ability to obtain within applicable timeframe
  • Possession of DEA registration or eligibility required
  • BLS certification and maintenance
  • Open to experienced or new graduate providers

Why Central Florida?
An exceptional blend of career opportunity, affordability, and quality of life awaits! Known for its natural beauty and welcoming community, the region provides a relaxed lifestyle without sacrificing access to major metropolitan areas. Residents enjoy scenic parks and springs, year-round outdoor recreations, excellent schools, and a cost of living that is often more affordable than Florida's larger coastal cities.
With convenient access to Orlando, Tampa, and Jacksonville, Central Florida offers the best of both worlds: small-town charm and easy access to world-class entertainment, dining, healthcare, and travel. Whether you're looking to grow your career, raise a family, or enjoy an active outdoor lifestyle, Ocala and Central Florida provide an ideal place to live and work.
If you're a Primary Care Physician looking to practice medicine differently - with the support, compensation, and mission alignment to do it well - we invite you to explore a career with Upperline Health.
