Primary Care Physician

Primary Care Physician (MD/DO) โ€“ Value-Based Primary Care Position Overview We are seeking compassionate, patient-focused Primary Care Physicians to join growing outpatient practices throughout the region. These opportunities are ideal for physicians who are passionate about delivering high-quality, value-based care to adult and senior patient populations in a collaborative, team-oriented environment. As a Primary Care Physician, you will provide comprehensive outpatient care with an emphasis on preventive medicine, chronic disease management, wellness, and building long-term relationships with patients. Physicians are supported by experienced clinical and administrative teams, allowing you to focus on delivering exceptional patient care while maintaining a healthy work-life balance. Responsibilities

  • Provide comprehensive outpatient primary care services to adult and geriatric patients.
  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Develop individualized care plans that promote preventive care, wellness, and disease management.
  • Coordinate care with specialists, hospitals, and interdisciplinary care teams.
  • Educate patients and caregivers on treatment plans, preventive health, and healthy lifestyle choices.
  • Maintain accurate and timely medical documentation within the electronic medical record (EMR).
  • Participate in quality improvement initiatives focused on value-based care and positive patient outcomes.
  • Build strong, long-term relationships with patients and their families through compassionate, patient-centered care.

Qualifications

  • MD or DO from an accredited medical school.
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active Florida medical license (or eligible for licensure).
  • DEA license (or eligible).
  • Current BLS certification.
  • Experience with Medicare Advantage, managed care, value-based care, or geriatric populations is preferred but not required.
  • New graduates are encouraged to apply.
  • Bilingual (Spanish) skills are preferred for select locations.

Schedule

  • Full-time Monday through Friday
  • Daytime outpatient hours
  • Minimal call requirements, depending on practice location
