Description: Nurse Responsibilities: Participate in the development and implementation of the service plan and initial assessments for clients receiving nursing services to ensure proper placement of staff. Regularly assesses the needs of clients receiving nursing services and complete service plans, documenting observation of the patient's condition at each visit. Ambulation, transfer, and positioning of clients; Observation and assessments; Wound care; catheter care; Ventilator care; IV therapy; lab blood draws; injections; Diabetic care/teaching; Colostomy care/teaching; Tracheostomy care; Internal/parenteral feedings care/teaching; Post op/recovery care; Medication administration teaching and monitoring. Coordinate with Physicians, Physical Therapist, Occupational Therapists, and other individuals in the patient's care plan. Educating patients and families on proper home health care procedures and strategies. Recommend tools or devices that may improve quality of life for the patients. Regular evaluations and reevaluations (as warranted by changes in condition but at least every 60 days) of the patient's nursing needs. Taking verbal orders. Make referrals to other disciplines, as indicated by patient's assessment. Conduct training as needed including but not limited to: Medication Administration, New Staff Orientation, In-service Training. Supervise CNAs and PCAs and Nurses performance and management of records. Investigate incidents/grievances & accidents. Communicate timely and effectively with agency on changes in visit assignment, dates of scheduled visits, schedule changes and patient care to ensure patient needs are met. Maintain training requirements as regulated by the governing authorities.

Nurse Requirements: 2+ years' work experience. Basic Health and Tuberculosis Screening. Participate in Quality Improvement Program. Requirement for job duties able to lift up to 50lbs. Current RN licensure for state of practice & Meet recurring training requirements. Current CPR (American Heart Association). Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment. Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
