 Registered Nurse (RN) – VA & Veteran Client Assessments

Company: Freedom Home Care

Client Population: Veterans & VA-Authorized Clients

Service Area: Cincinnati & Surrounding Areas (East & West Cincinnati, Ripley, Hillsboro, Chillicothe, Hamilton, Dayton)

Position Type: Full-Time | 40 Hours/Week | Salaried

Schedule: Flexible – Self-Scheduled Visits

About The Role

Freedom Home Care specializes in providing in-home care services to veterans and their families. We are seeking a dependable, compassionate, and self-directed Registered Nurse (RN) to complete VA-authorized assessments, reassessments, care planning, and follow-up visits for new and existing veteran clients.

This role offers flexibility within a full-time salaried structure. You will manage your weekly workload, schedule visits directly with clients, and travel to veterans’ homes throughout our service area.

Responsibilities

  • Complete initial in-home assessments for veterans starting services
  • Perform required VA reassessment visits for ongoing clients
  • Create, update, and revise client care plans based on assessment findings and VA guidelines
  • Conduct post-hospitalization visits when veterans return home
  • Educate caregivers on the client’s care plan and any care plan updates
  • Provide caregiver skill education as needed, either on a case-by-case basis or in group settings
  • Assess client condition, safety, and ongoing care needs in accordance with VA requirements
  • Review client information prior to visits using our care management platform
  • Communicate assessment findings and care plan updates with the office care coordination team
  • Contact VA nursing staff as needed to coordinate follow-up appointments after hospitalizations
  • Provide professional recommendations to the VA for increased service hours when supported by assessment findings
  • Accurately document all assessments, care plans, education provided, and recommendations in a timely manner
  • Maintain professionalism, confidentiality, and VA compliance at all times

Requirements

  • Current and active Registered Nurse (RN) license in the State of Ohio
  • Experience with home care, veteran care, or VA-authorized services preferred (but not required)
  • Ability to work independently and manage a flexible full-time schedule
  • Reliable transportation and willingness to travel to client homes
  • Strong assessment, communication, and documentation skills
  • Must successfully complete a federal background check and drug screening prior to employment
  • Ability to confidently use a smartphone and mobile applications for scheduling, communication, and documentation
  • Access to a computer with the ability to navigate online platforms and use Excel to review client information
