POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordinates patient care across the continuum, conducts utilization review to ensure appropriate level of care and resource use, and facilitates safe, timely discharge planning by collaborating with patients, families, physicians, and the interdisciplinary healthcare team.

MINIMUM REQUIRED QUALIFICATIONS

  • ME/compact RN license
  • BLS
  • 4-5 years RN experience
  • 3+ years of acute clinical experience, not just Case Management experience
  • Prior completed travel assignments
  • 2-3 prior Case Management travel assignments completed
  • Care Coordination, Discharge Planning, and Utilization Review experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7:30-4:00p. Every third weekend 7:30-4:00. 40hrs/wk.

START DATE – ASAP
