Psychiatric Mental Health Nurse Practitioner (PMHNP)

Deer Oaks Behavioral Health - Long-Term Care Clinical Services

In this role, you will be assigned to a facility or facilities in your local community, allowing you to build meaningful connections with the individuals you serve. You will have the opportunity to conduct in-person psychiatric assessments, medication management, and implementing Medicare compliant gradual dose reduction strategies for psychotropic medications. We understand the importance of geriatric-focused training and continuous professional development, which is why we provide accredited CEUs and ongoing case consultation with our experienced team of clinicians. Clinical Care
  • Conduct comprehensive psychiatric evaluations and mental health assessments for LTC residents
  • Develop, implement, and adjust individualized treatment plans
  • Prescribe and manage psychotropic medications in accordance with Medicare and LTC best practices
  • Implement and document Medicare-compliant gradual dose reduction (GDR) strategies
  • Monitor and treat chronic mental health conditions including depression, anxiety, bipolar disorder, schizophrenia, and dementia-related behavioral disturbances
  • Collaborate with interdisciplinary teams - nursing, medical providers, social services, rehabilitation, and facility leadership
  • Complete accurate documentation of assessments, treatment plans, and progress notes using the electronic medical record (EMR) system
Interdisciplinary Collaboration
  • Work closely with nursing, social services, rehabilitation, and medical providers to ensure coordinated psychiatric care.
  • Participate in care plan discussions, facility meetings, and case collaborations as needed.
Documentation & Compliance
  • Maintain complete and timely documentation using electronic medical records (EMR).
  • Follow ethical, professional, and regulatory guidelines for psychiatric documentation and care delivery.
Facility Partnership
