Work Environment

This role will require you to work onsite, primarily in patients' homes, which provides a unique opportunity to deliver personalized physical therapy services in a comfortable and familiar environment. Key Responsibilities

• Conduct thorough evaluations by reviewing patients' medical histories, performing specific assessments, and determining suitable treatment recommendations tailored to individual needs.
• Communicate evaluation results and treatment plans clearly to patients and their families, actively involving them in the goal-setting process.
• Implement evidence-based treatment protocols that address the patient's presenting condition while adapting to changes in their health care needs.
• Collaborate with other healthcare professionals as appropriate, documenting any changes in the patient's condition that warrant adjustments to their care plan.
• Develop and execute comprehensive discharge plans, ensuring continuity of care through detailed documentation including initial evaluations, ongoing progress notes, and final discharge summaries.
• Manage your schedule effectively to meet productivity expectations, displaying punctuality for meetings and other engagements, and contributing to community outreach efforts.
• Contribute to departmental goals and compliance with all applicable regulations and standards to maintain quality of care.
• Engage in ongoing professional development, taking initiative in continuing education opportunities and serving as a clinical instructor to students.
• Enhance patient experience by being adaptable and responsive to their needs, actively promoting access to care through participation in quality improvement initiatives.

Patient Population / Client Focus

In this role, you will primarily serve patients in their homes, ranging from individuals recovering from surgeries to those managing chronic conditions. Your empathetic approach and expertise will empower these patients to regain mobility, improve function, and enhance their overall quality of life.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• State licensure as a Physical Therapist, in compliance with Virginia state regulations.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

• A minimum of 2 years of relevant experience in physical therapy practice is preferred, although candidates with lesser experience are encouraged to apply.

Compensation and Benefits

The salary range for this full-time position is competitive, offering between USD 100,000 and USD 120,000 annually, commensurate with experience. In addition, we provide a comprehensive benefits package, including:
