Reports To: RN Clinical Supervisor
JOB DESCRIPTION SUMMARY
The Clinical Coordinator is responsible for the day-to-day planning, coordinating, managing and directing all activities as they relate to the Management of the hospice patients. The Clinical Coordinatorparticipates the IDT meetings and supervises and supports clinical staff through evaluation and fieldsupervisory visits and assures ongoing development of the teams' clinical skills. The ClinicalCoordinator will assure quality care and compliance with state and federal regulations, ACHCaccreditation standards, and the organization's policies and procedures.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Quality Provision of Services
- Receives/triages inquiries from the staff, patients, caregivers, and the community. Performs telephone assessment to determine needs of the caller. Develops and implements a plan to address the needs by (but not limited to): instruction, dispatching appropriate staff to make a patient visit, or contacting a physician.
- Assures that members of the interdisciplinary team have demonstrated the skills competenciesto care for assigned patients.
- Assists the clinical supervisor as a liaison between Hospice and other professionals, includingthe HMD, community physicians, hospitals, nursing homes, and agencies.
- Enhances the integrity and clinical practice of the team by providing them with a consistentrole model, mentor, instructor and resource person.
- Assumes responsibility for own professional growth and development to maintain andimprove competence.
- Evaluates own needs for support and uses identified system(s) to meet the need.
- Develops new skills by participating in ongoing education and maintaining knowledge of current nursing practice through journals, literature review, etc.
- Participates in quality improvement activities, QAPI program and hospice sponsored in-servicetraining. Serves on committees or teams as approved by the clinical supervisor.
Organizational/Regulatory Compliance
- Ensures compliance with all areas of federal, state and local laws and regulations as well asACHC accreditation standards, Conditions of Participation, and the organization's standards ofcare, policies and procedures.
- Communicates with the appropriate physician(s) regarding the patient's condition,incorporating hospice expertise. Implements physician's orders by documenting and communicating to staff, patient/caregiver or facility. Utilizes appropriate resource services(ambulance, IV therapy, DME, inpatient facilities, and appropriate agencies) to implementorders.
- Performs the introductory (90 day) and annual performance review of assigned team membersin accordance with the organization's policy and timelines.
- Assures that licensed practical nurses, nursing assistants and other care assistants work underthe direct supervision of a registered nurse.
- Supports and assures that clinical documents meet the organization's patient care andreimbursement regulatory standards. This is accomplished through use of tools such as: theAdmissions Audit tool, IDT meetings and documentation.
- Assures completion of appropriate documentation and inclusion of a copy in the facility chart.
- Follows all rules and regulations of the facility.
People/Communications
- Assigns and coordinates daily schedules of clinical staff, including PRN staff. Facilitates theprovision of patient care services on a day-to-day basis, according to the plan of care. Worksclosely with team members to meet agency productivity standards and patient care needs.
- Coordinates all interdisciplinary team services, including on-call personnel, in conjunction withthe After-Hours Department in order to assure 24-hour availability of patient care services.
- Assists the clinical supervisor to assess the need for new staff and to recruit, interview, andmake hiring decision in collaboration with Human Resources. Orients new staff to the teamafter HR and clinical orientations.
- Coordinates with the clinical supervisor to assure that assigned staff attend andreceive education programs.
- Coordinates with staff in contract bed facilities regarding the hospice plan of care, standingorders, etc. Assists with the implementation of a plan to address identified needs.
- Collaborates and communicates with the facility staff and CNA regarding patient status and thecare provided.
- Fulfills any after-hour supervisory/on-call support assignments as assigned by DON/Clinical Supervisor.
Financial Stewardship
- Assures accuracy and timeliness of clinical staff day sheets and time sheets.
- Collaborates with the clinical supervisor to provide assessment, planning, implementation andevaluation of team goals.
- Assists the clinical supervisor in creating the clinical budget. Shares responsibility with the clinical supervisor in adhering to the budget. Creates action plans to addressand resolve budget variances.
- Achieves expected productivity standard as defined by the organization and in alignment withbest practices/the Hospice of Acadiana model.

- Licensure/Certification/Registration: Must have nursing licensure from the governing body for nurse practice for Louisiana
- Education: Must have at least a RN from a NLN accredited nursing program. A BSN ispreferred.
- Experience: At least two (2) year of Hospice/Home Health experience preferred. A minimumof one year of management experience preferred and at least four years of nursing experience.
- Possesses current Louisiana Driver's License, Automobile Liability Coverage and CPR cert.
- The ability to demonstrate superlative oral and written communication skills as well as ability. toorganize and manage projects within defined timeframe and budget.
