Job Overview:

Responsible for promoting appropriate Length of Stay (LOS) and resource management in areas assigned; assesses the care needs of patients upon entry into the organization and facilitates appropriate progression through the hospital's continuum of care into the post-hospital environment; collaborates with other members of the health care team in planning, coordinating, and facilitating the implementation of a plan of care based on the reason for admission and current, emerging and projected care needs; performs these duties in a manner which enhances service excellence including quality clinical outcomes for patients, efficiencies in hospital operations, and cost-effectiveness; and demonstrates accountability for services provides through participation in the monitoring and evaluation of patient care activities.  Other duties as assigned.

General Responsibilities:

Qualifications
  • A bachelor's degree in a health care related curriculum is preferred.  Candidates must possess at least a bachelor's degree from an accredited program.
  • A minimum of three (3) years’ experience in direct patient care in an acute care setting; additional experience in Case Management/Utilization Management/Discharge Planning is required.
  • Nurse candidates must hold a current licensure as a Registered Nurse in the state of Florida.

  • BLS is recommended, but not required.
