UF Health Spanish Plaines Hospital, formerly known as The Villages Regional Hospital and acquired by University of Florida Health in January 2020, has been serving residents of The Villages as well as patients in Lake, Sumter and Marion counties since 2002. We are part of UF Health Central Florida, a not-for-profit health care system and the largest most comprehensive provider of health care services in the region. UF Health Central Florida provides inpatient acute hospital services at UF Health Spanish Plaines Hospital and UF Health Leesburg Hospital, inpatient rehabilitation services at UF Health Rehab Hospital - The Villages and diagnostic laboratory services at several locations. Responsibilities
The Case Manager utilizes professional nursing skills to assess patient and family scenarios for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care. Assessment, planning, intervention and reassessment are performed in a collaborative environment. The key goals of the Case Manager include proactive, individualized planning for patient's progress that optimizes quality of care, utilization, cost and patient satisfaction.
Qualifications
Education:
- Bachelors Degree Preferred
- Diploma/Associates Degree in Nursing
- Professional Certification in Case Management, preferred
Licensure/Certification/Registration:
- Florida Registered Nurse License
Special Skills/Qualifications/Additional Training/Experience Required:
- Effective communication skills to facilitate contact with physicians, patients/clients, families and staff.
- Minimum of 2 years experience, Case Management experience preferred.
- CCM certification preferred.
- Must be able to read, write, speak and understand English
