Job description

· Deliver agency health education programs for target populations

· Develop relationships with community support services for collaborative outreach and referral

· Mentor and assist in training of community-based interns

· Update and translate presentations and/or handouts, or create new ones as needed

· Schedule services, events such as health fairs, presentations, and other activities in the community

· Conduct lung-health screenings, including spirometry, blood pressure and oxygen saturation screening

· Conduct clean indoor air quality home assessments (and fall prevention for seniors)

· Provide tobacco cessation information and assistance

· Distribute respiratory equipment and lung disease remediation supplies

· Prepare progress reports and program evaluation

  • Resident of San Mateo, San Francisco, or South Santa Clara County, or East Bay preferred
