Job description

JOB DESCRIPTION - QIDP - QUALIFICATIONS:
 
Must be pleasant, dependable and have good judgment and leadership abilities. Must demonstrate maturity. Must have knowledge of the normalization process. Must be a good organizer with the ability to implement plans and delegate. Must be able to learn new techniques easily. Must have a Bachelor's degree or higher in Psychology, Sociology, Human Development, Liberal Arts in Child with Disabilities or Social Services. In addition, must have at least 1 year experience working with Developmentally Disabled persons and qualify according to federal regulations. These requirements must be met and approval granted from the Department of Developmental Services.
 
JOB SUMMARY:
 
Under the general supervision of the Administrator/owner, the QIDP's main focus is on the habilitative, social and recreational nature of the Developmentally Disabled resident. These activities will meet the needs and interests of the resident group, will promote independence and shall involve those resources which may maintain and encourage supplemental programming of the residents. In addition, he/she is responsible for the development and implementation of program services/active treatment and supervision of the program staff and direct care staff.
 
Hours: 7:30AM-4:00PM, M-F
 
RESPONSIBILITIES:
 
Chair ISP meetings as part of the Interdisciplinary team. Generate ISP packets, make sure they are together for meetings and ensure that consultants have completed their part before the meeting.Complete page 5 for the ISP meeting.
Develop new forms when needed.
Complete quarterly assessments and put them in the chart..Complete schedules annually for ISP meeting, quarterlies.
Complete Target Date Sheets
Chair Human Rights committee, Infection Control, Safety and Pharmacy quarterly meetings. Complete minutes for the above meeting quarterly.
Be available for staff, client, owner or facility concerns 24 hours per day 7 days per week.
Make certain staff and clients are safe and free from harm.Write letters when needed for Regional, Staff issues or legal issues.
Participate in the survey process given by the Department of Public Health.
Completes all Plan of Corrections given by the Department of Public Health.
Make sure program equipment is set up in the appropriate room at the right time and ensures someone is there to implement, lead and direct others in their designated program.
Responsible for hiring as needed, completes disciplinary action and dismissal of direct care staff. Make random rounds of programming rooms & dining rooms intermittently for supervision of program objectives.
To develop the skill and knowledge necessary to enhance development of each residency within the scope of specific programming areas so each resident can attain their highest level of functioning.
Take data from data collection sheets monthly and enter the frequencies on the graph in the chart as well as check to ensure reliable data.
Put new data collection in the group books each month.
Help teach assigned personnel the procedures, techniques and methods being utilized in the designated program.
In-service the Direct Care Staff if needed.
Supervise Multi-Disciplinary professional staff by making sure reports/objectives are current and appropriate. If not let them know and/or discuss at ISP.To see that assigned personnel understand their duties, and schedules and program goals.
To assist direct and teach assigned personnel training in accordance with policy procedures and techniques established by the facility.
To have the activity director order appropriate equipment and supplies in the program rooms, in accordance to the recommendation of other staff and observation.
Ensure activity areas are kept clean and neat.To give input to the ID team as necessary.
Assure clients privacy and monitor hygiene, ie: shaving, teeth brushing, no body odor.
Make sure clothes are appropriate, well-fitting and appropriate for weather. Notify the social Services Designee if they are not.
Make sure clients aren't left unattended.
Complete direct Care Staff evaluations with supervisors.
To carry out other duties as specified by the Administrator/Owner to the highest degree of their personal ability.
Interact with parents, clients, teachers, workshops and Regional center personnel individually or as part of the ISP process.
Create new graphs annually.
Go to the client’s school, day program and/or workshop monthly.
Assist Psychiatrist monthly and give input on client's behaviors/seizure activity etc.
Type the HS231's to submit to SCLARC and to go with the TAR's. Submits packets to SCLARC for consent of psych meds/dental/mammograms etc.
Make out the quarterly lists for the Activity Director for her to add the activities.
Prints sleep logs for CNA's/LVN/PM Supervisor.Make out employee evaluation forms for department head use.
Confirms the validity of certificates for all CNA's hired and prints for DSD.
Makes sure consultant staff/RN/Trident lab/Pharmacy/Doctors licenses are current.
Assess clients for possible admission with the DON & Doris
Updates the Program Plan annually for approval by the Department of Developmental Services.
Completes the 2nd half of new hire orientations after the SDS does his
Act as Administrator in the case the actual Administrator is gone for more than 10 consecutive days..
Maintain a file of current contracts with outside agencies and change when needed.
Supervise during annual licensing inspection process
Update and keep current publication files
Meet with families and residents in grievance issues. Establish appropriate procedures for excellence in operation of the facility.
Attend and chair all monthly and quarterly UR meetings. Represent facility at professional functions. Interface with consultants to provide quality assurance and consistent care of the resident population.
Assist Department Heads in counseling employees and terminate when needed.Interpret the facility and its services to patients, relatives, visitors and the community.
Meet with purveyors and providers to solve problems when and if they occur.
Type and prepare ISP copies for day program, parents and Regional Center Make out employee evaluation forms for department head use. Take identification pictures
