Position Full-time - 8 hour shifts/40 hour per week
Wage Type: Non-Exempt
Reports To: Clinical Director 
Position Summary:
The Triage Nurse provides telephonic and clinic-based nursing support for patients with bleeding disorders in an outpatient setting. In the Triage role, the nurse responds to incoming calls and patient messages from multiple access points, addresses incoming pharmacy faxes, processes medication refill requests, and prioritizes emergency department referrals using established clinical protocols and sound nursing judgment.
In the Outpatient Infusion Nurse role, the nurse will specialize in administering IV therapy, including medications, and fluids to patients with bleeding disorders in an outpatient setting. The ideal candidate has exceptional vascular access skills, a strong foundation in pharmacology, and the ability to provide personalized, patient-centered care, ensuring safety and comfort throughout their treatment journey.
In the Clinic role, the nurse supports efficient clinic operations through routine clinical tasks, proactive problem-solving, and close collaboration with the interdisciplinary care team. The position emphasizes patient education, care coordination, and accurate, timely documentation in EPIC to ensure safe, seamless, and patient-centered care. 
Education/Criteria:
Graduate of accredited nursing program with Associate Degree, or BSN. Nurse Coordinator must be licensed as a Registered Nurse by the State of Wisconsin or reciprocal state. Must have and maintain CPR for healthcare provider certification. 
Preferred Knowledge, Skills, and Abilities: 
  • At least 3 years of general nursing experience required
  • IV infusion experience required
  • EPIC experience required
  • Hospital nursing experience preferred
  • At least 1 year clinic experience preferred
Expectations:
Needs to be self-motivated and able to complete work independently. Also needs to work cooperatively with others in a multi-disciplinary team environment. Strong organizational and professional communication skills are necessary. 
Responsibilities/Duties:
Triage RN responsibilities:
  • Clinical Assessment & Triage: Receive and respond to high-volume patient calls and messages, performing focused assessments to determine symptom severity and triage priority (acute vs. routine).
  • Patient Advice & Education: Educate patients on home care techniques, medication management, and symptom management.
  • Care Coordination: Act as a liaison between patients and physicians/APP providers, communicating clinical concerns promptly.
  • EMR Documentation: Accurately document all patient interactions, assessments, and advice in the Electronic Medical Record (EMR) system, ensuring compliance with clinic protocols.
  • Protocol Management: Apply, and adhere to, physician-approved, evidence-based triage protocols to recommend appropriate care levels (e.g., home care, urgent care, or emergency room).
  • Result Reporting & Follow-up: Communicate lab/test results to patients as directed by providers and follow up on pending tests.
  • Prescription Management: Process patient requests for medication refills and prior authorizations following established, provider-approved guidelines.: 
 Infusion RN Responsibilities:
  • Assessment & Care Planning: Perform thorough pre-infusion assessments, review laboratory results, and develop individualized care plans.
  • IV Therapy Administration: Safely initiate, maintain, and discontinue peripheral IVs, PICC lines, and implanted ports. Administer chemotherapy, biotherapy, antibiotics, blood products, and other specialty medications as ordered.
  • Patient Monitoring: Monitor patients throughout infusion sessions, watching for adverse effects, allergic reactions, or complications like infiltration and infection.
  • Emergency Response: Respond promptly to anaphylaxis or side effects, collaborating with physicians and pharmacists to manage complications.
  • Patient Education: Instruct patients and families on medication side effects, treatment plans, and post-infusion care.
  • Documentation & Compliance: Maintain accurate, timely Electronic Health Record (EHR) documentation. Adhere to strict infection control, ONS chemotherapy guidelines, and regulatory standards.
  • Collaboration: Work within an interdisciplinary team to coordinate care and maintain a calm, welcoming environment.
Position Type and Expected Hours of Work:
This is a full-time position in a health care facility open Monday through Friday except for holidays recognized by the Hemophilia Outreach Center. Expected hours for this position are 8:00am - 4:30pm. Provides 24/7 on-call care when clinic is closed 4-5 days every other month with 1-2 holidays per year. Some additional hours are possible on an as needed basis depending on schedule of events, operational demand, and/or patient demand.
