Job description

On-Call Registered Nurse (RN) Shifts: 7 on, 7 off - Home Health & Hospice
Auburn Crest Home Health & Hospice is seeking a compassionate and dependable On-Call Registered Nurse (RN) to join our team serving the Salt lake Valley. This role includes after-hours telephone triage from 5:00 PM to 8:30 AM, providing in-home visits as needed, documenting and communicating patient updates to case managers, and supporting patients and families during critical moments of care.
We are looking for a nurse who is confident working independently while also collaborating closely with our interdisciplinary team to provide exceptional home health and hospice care.

Responsibilities
  • Provide skilled nursing care to home health and hospice patients in their homes
  • Respond to after-hours patient and family needs through telephone triage
  • Complete nursing visits as needed during evening and overnight hours
  • Accurately document care and communicate updates to the clinical team
  • Support admissions and urgent patient needs when necessary
  • Collaborate with the Director of Nursing and interdisciplinary team to ensure quality patient care

Qualifications
  • Current Utah Registered Nurse (RN) license
  • Minimum of one year of nursing experience; home health or hospice experience preferred
  • Current CPR certification
  • Strong communication and documentation skills
  • Ability to work independently and make sound clinical decisions
  • Schedule flexibility (this position requires weekday evening/night time, weekend, and holiday availability) and travel required
  • Reliable transportation, valid driver's license, and current vehicle insurance
