Insight Behavioral Health is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our expanding team. This remote role allows for flexibility and promotes a strong work-life balance. You will collaborate with an interdisciplinary team of professionals, including psychiatrists, therapists, and case managers, convening regularly to discuss case updates and share insights that help optimize patient care. Your workspace will include various digital tools and telehealth platforms that support effective communication and care management from the comfort of your home.

• Provide emotional support and encouragement to patients and their families during behavioral health crises, ensuring they feel heard and supported.
• Assess and manage case referrals, identifying needs and care gaps, and facilitating a structured approach to each patient’s treatment plan.
• Guide patients and families toward achieving individual recovery goals through psychoeducation, connecting them with relevant community resources and support services.
• Evaluate social determinants of health affecting patient care, including housing, food insecurity, transportation, and income, and develop strategies to address these barriers.
• Create connections for patients and families with support groups, community networks, and resources to foster mental wellness and recovery stability.
• Work closely with interdisciplinary team members to implement and refine treatment plans, ensuring alignment with patient and family goals.
• Participate in regular consultations with your team, reviewing patient progress and adapting approaches as necessary to enhance care outcomes.

The individuals you will support are typically experiencing complex behavioral health issues, including severe mental illness, substance use disorders, and acute mental health crises. Your expertise will not only aid these patients but also empower their families, ultimately working towards holistic recovery for all involved.

• A Master’s degree in Social Work (MSW) is required.
• An active and valid license to practice as a Licensed Clinical Social Worker in the state of California is mandatory.
• Strong communication and interpersonal skills, with the ability to build rapport and trust with patients and families.
• Proficiency with telehealth systems and electronic health records (EHR) for effective patient management.

• At least 2 years of experience in team-based care environments, ideally in outpatient settings.
• Familiarity with child and adolescent case management is advantageous.
• Experience in addressing acute behavioral health needs such as suicidality, aggression, or chronic mental illness is highly valued.
• A deep understanding of community resources relevant to mental health care and recovery support.
