Job description


Role and Responsibilities

Provide personal care and supportive services to individuals in the community, in the privacy of their home.

Essential Functions

  • Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.
  • Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
  • Immediately report changes in client’s condition to supervising professional.
  • Ensure the safety of the client, client’s family and/or the employee.
  • Maintain HIPAA.
  • Complete and submit necessary patient documentation including charting tasks and observations on android device.
  • Attend ongoing in-service programs in a timely manner, to maintain certification.
  • All other duties as assigned.

Education Requirements

  • High School diploma or GED preferred.
  • Completion of an approved program of PCA or HHA certification as designated by NYS Department of Health.

Qualifications and Requirements

  • Valid Driver's License and reliable transportation.
  • Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program. )
  • Must have worked in a certified HHA or PCA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry).

Work Environment

The HHA/PCA is primarily in a non-office setting and may be exposed to outdoor conditions.


The working conditions are classified as medium work:

  • Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
