As a Case Manager, you will serve as a trusted partner and advocate for Veterans as they navigate housing, healthcare, employment, benefits, and other essential services. Through a trauma-informed and Veteran-centered approach, you will help individuals identify goals, overcome barriers, and build a foundation for long-term stability.
What You'll Do
Build supportive relationships with Veterans and provide individualized case management services.
Conduct assessments and develop personalized service plans that promote housing stability and self-sufficiency.
Connect Veterans with healthcare, behavioral health, substance use treatment, employment services, benefits, and community resources.
Support Veterans in securing and maintaining permanent housing.
Collaborate with VA partners, community providers, landlords, and internal Harbor Care programs to coordinate services.
Advocate for Veterans and help remove barriers to housing, healthcare, and other critical supports.
Maintain accurate documentation and ensure compliance with VA Grant and Per Diem (GPD) program requirements.
Participate in team meetings, case reviews, and collaborative planning efforts.
Help create a safe, respectful, and welcoming environment where Veterans feel supported and empowered.
