Job description

Position: Transition Support โ€“ Residential Programs (PT)
Reports To: TLP Program Manager โ€“ Residential Programs Department: Residential Programs
Location: Tucker, GA
FSLA: Non-Exempt
Job Summary
The Transition Support provides direct support to the Life Coach and Transition Support Team and maintains the community living skills and support for young adults as they develop a higher level of independence. The Transition Support role typically provides services to youth on a day to day basis that support permanency, safety and well-being. The position also supports clients with life skills training, watchful oversight, support and guidance so they may attain a level of independence to live on their own.
Responsibilities
โ€ข Supervises/monitors youthโ€™s progress, based on youthโ€™s skills development and level of maturity as outlines in the clientโ€™s Supervision and Independence and Safety and Crisis management plans
โ€ข Orient new clients upon entry in the program
โ€ข Work in partnership with the Wellroot Life Coaches and other staff to develop and implement a plan for clients
โ€ข Teach and support individualized life skills according to the ISSP, CLSA results, and based on youthโ€™s need. Life skills including but not limited to; grocery shopping, self-care, daily living tasks, communications, budgeting, community living and socialization and community mobility
โ€ข Document at least four (4) efforts weekly individualized independent living skills development activities for each youth on assigned Transition Support Team that are aligned with the ISSP goals
โ€ข Offer job search training in areas such as resumeโ€™ writing and interviewing
โ€ข Provide budgeting and daily living skills by supporting and monitoring weekly budgeting โ€ข Provide applicable phone numbers to TLP youth in order to ensure twenty-four (24) hour access to appropriate personnel within the agency
โ€ข Appropriately maintain keys to each TLP youthโ€™s housing to ensure twenty-four (24) hour access to all TLP residences
โ€ข Ensuring and enforcing program expectations to ensure safety, growth and life skill development of the youth in the program โ€ข Assist clients in establishing and maintaining involvement in community recreation and productive leisure time activities
โ€ข Encourage the spiritual growth of each client by connecting them with spiritual resources on staff or in the community
โ€ข Teach within an evidence informed practice to encourage and supervise the reduction/elimination of maladaptive behaviors and follow individualized skills and service plans for clients
โ€ข Ensure that supervision is designed to allow the agency observation of the youth is practicing healthy life skills and sound decision-making
โ€ข Participates in service meetings with other personnel as appropriate
โ€ข Maintain required RBWO standards of supervision of residents at all times
โ€ข Support and maintain an environment of emotional and physical safety for clients
โ€ข Documents all reportable and non-reportable incidents that occur with clients and notify the appropriate parties within timeframe outlines in program policy
โ€ข Increase the clientโ€™s social and financial independence resulting in decreased oversight to ensure self-sufficiency โ€ข Provide schedules, routines environments and engagement of youth in the program
โ€ข Assist clients in obtaining medical services
โ€ข Transport clients to necessary medical, dental, mental health, educational appointments and job interviews/career fairs
โ€ข Transport youth to or from work or school when necessary
โ€ข Complete required record-keeping and documentation accurately and in a timely manner
โ€ข Maintain confidentiality of client information
โ€ข Maintain appropriate professional boundaries โ€ข Utilize a percentage of hours on assigned caseload as determined by supervisor
โ€ข Maintain required certifications and training hours
โ€ข Adhere to the organizationโ€™s policies and procedures, and demonstrate support of the mission, vision and values โ€ข Administer life skills assessments at prescribed intervals
Requirements
โ€ข 21 Years of age or older
โ€ข High school diploma or equivalent is required; a Bachelor of Science degree from an accredited college or university in the area of human services, social work, non-profit management or behavioral health is preferred; Masterโ€™s Degree in Social Work also preferred
โ€ข Minimum two years of work experience in a State of Georgia-regulated child welfare program is required; experience working with older adolescent population preferred
โ€ข Possess a valid Georgia driverโ€™s license
โ€ข Computer skills including basic proficiency and experience working with Microsoft Outlook, Word, Power point, Excel and ability to perform data entry tasks
โ€ข Be familiar with and perform duties in accordance with RBWO (Room Board and Watchful Oversight) standards.
โ€ข Subject to flexible hours, including split shifts or varied days and times, including mornings, afternoons, evenings, and weekends, depending on program needs
โ€ข Regularly travel in the community, drives an agency-owned vehicle, and transports clients
โ€ข Must participate in at least twenty-four (24) hours of annual training. At least twelve (12) hours should be directly related to work with teens including understanding developmental needs of adolescents and strength-based assessments
โ€ข Willing to enthusiastically support the mission, vision and values of Wellroot Essential Demonstrated Skills and Abilities:
โ€ข Ability to develop and maintain professionally appropriate therapeutic relationships with each client; ability to maintain positive working relationships with staff and other related persons
โ€ข Ability to complete a needs assessment of clientโ€™s independent living skills
โ€ข Understanding of service planning and person-centered planning and delivery โ€ข General knowledge of social services, record keeping, reporting requirements and confidentiality
โ€ข Ability to read, write and speak the English language at a level that meets the performance requirements
โ€ข Ability to make independent decision when circumstances warrant such action โ€ข Ability and willingness to maintain a positive demeanor
โ€ข Ability to effectively communicate with others
โ€ข Willingness to respect the lifestyle choices of others, including but not limited to sexuality, religion, and political affiliation
โ€ข Ability to implement any therapeutic intervention as required
โ€ข Consistent demonstration of emotional stability
โ€ข Ability and willingness to follow deadlines and/or complete assignments/trainings as assigned by supervisor
